COVID Letter Sent to Impacted Groups
Dear Thomas J Waters Elementary School Parent and/or Guardian, 

Chicago Public Schools (CPS) is committed to keeping our school communities safe from COVID-19. The District was recently made aware that one or more person(s) in classroom ____ at Thomas J Waters Elementary School has tested positive for COVID-19.

The Chicago Department of Public Health (CDPH) and CPS are highly encouraging your student in {The impacted group- ie A309, sports team, etc} to wear a mask for 10 days from the latest exposure, until 2023-xx-xx. 
Since your student was identified as being potentially exposed, CPS offers and highly encourages rapid testing; your student will receive one testing kit containing two COVID-19 rapid tests from the school. 

Please test on the next immediate Monday and Thursday using a rapid test. Please follow the instructions on the test package. If your student tests positive, they should report the case to CPS at https://cps.edu/covidresults and isolate at home for 5 days following the positive test. Masking is REQUIRED upon returning to school from isolation for an additional 5 days.

As always, please monitor your student for COVID-19 symptoms. If symptoms arise - even if they are mild - please contact your healthcare provider, keep them home from school, and get tested for COVID-19 right away. To find a test site near you, visit chicago.gov/covidtesting. If your student tests positive for COVID-19, please report their case at cps.edu/covidresults and they must stay home for at least five days from when symptoms began. 

The best way to keep you and your student safe from COVID-19 is to get vaccinated. Vaccines are safe, effective, and available to everyone ages six months and older. To find a vaccine appointment, search http://vaccines.gov, text your ZIP code to 438829, call 1-800-232-0233, or visit cps.edu/vaccinations. If you need help locating medical care or have any CPS-specific questions, please visit cps.edu or reach out to the Healthy CPS Hotline at 773-553-KIDS (5437), which is staffed 9 a.m. to 5 p.m., Monday through Friday.

Las Escuelas Públicas de Chicago (CPS) se comprometen a mantener informada a nuestras comunidades escolares sobre los desarrollos con respecto al COVID-19. El Distrito se enteró recientemente de que más de una persona en el aula número A309 de Thomas J Waters Elementary School ha dado positivo por COVID-19.

El Departamento de Salud Pública de Chicago (CDPH) y las CPS recomiendan que su estudiante miembro del {The impacted group- ie A309, sports team, etc} use mascarilla durante diez días, a partir de la fecha de su última exposición y hasta el 2023-09-03.
