Enrollment
Waters Elementary School is a neighborhood elementary school, with programs specializing in fine and performing arts, ecology and technology. We have copies of all enrollment forms in the main office.

CPS Required Documents:

Proof of AgeProof of age includes, but is not limited to, one of the documents listed below:
  • Child’s original birth certificate
  • Child’s baptismal record (not accepted for preschool enrollment)
  • U.S. or Foreign Passport
  • Court documents (accepted for preschool enrollment — foster children only)
  • Medical records (not accepted for preschool enrollment)
*State-issued medical card also accepted for preschool enrollment only.
Proof of Current AddressProof of current address includes, but is not limited to, any two of the documents listed below:
  • Current utility bills
  • Illinois driver’s license or State of Illinois identification card
  • Deed
  • Employee identification card
  • MediPlan/Medicaid card
  • Voter registration card
  • Court documents
  • Illinois Department of Public Aid card
  • Stamped United States Post Office change of address form
  • Illinois state aid check/social security check
  • Other identification card issued by a federal or state agency or a foreign government consulate, such as a Matricula Consular

Medical Compliance

Medical Compliance Immunizations and physicals are required for students entering ninth grade. It is recommended that students receive their immunizations and physicals prior to the end of their eighth grade year. View the Health Forms for Parents and Caregivers page for specific forms.
Physical Examination RequirementsAll students must have a physical examination within one year of:
  • entering schools in the State of Illinois for the first time at any grade level
  • entering kindergarten or first grade, fifth grade, and ninth grade (ages 5, 11, 15 for ungraded programs)
  • entering preschool up to age 6 (physical exam and lead screenings)

  • Diphtheria, Pertussis (Whooping Cough), Tetanus (DTP/Td)
  • Inactivated Polio
  • Measles, Mumps and Rubella (MMR)
  • Hepatitis B
  • Varicella (Chicken Pox)
  • Haemophilus Influenza, Type B (HIB)
  • Conjugate Meningitis (MCV4)
Recommended Vaccine
  • HPV (human papillomavirus)
Dental RequirementsAll students in kindergarten, 2nd, 6th, and 9th grade must have a dental exam completed by a licensed dentist prior to May 15th of the current school year.
Pre-K RequirementsOnce you have accepted an offer for placement in a CPS-managed preschool program, the school will communicate enrollment information. When you visit your chosen school to register your child, bring the following documents: 
  • Proof of child’s age
  • Proof of current Chicago address
  • Health forms, including physical examination, immunization, and dental

PreK Applications

CPS full-day programs serve 4 year olds only (A child is considered age 4 if they turn 4 years old on or before 9/1/2022). If your child is 3 years old, please select a Community Based Early Learning Program or a CPS half-day program on the Locations page.

Application Assistance
If you have questions or are in need of assistance when completing the application, call 312-229-1690 for assistance with applications for both CPS schools and Community-Based Preschools. You can also visit any of the Family Resource Centers by appointment only located around the city, or visit a Community Early Learning Provider if you are interested in a site.

Neighborhood Enrollment

If you live within the attendance area and your child is in kindergarten or older, you are automatically guaranteed admission to our school. Registration is mandatory and proof of your child's age and two documents proving your address are required.

GoCPS (Outside Attendance Area)

Waters can accept students who live outside the attendance area through the CPS Office of Access and Enrollment computerized lottery. Applications will be due in early December for the coming school year.

Due to the unknown number of incoming kindergarten neighborhood students, all kindergarten lottery students are waitlisted in two categories: sibling and general lottery.

Acceptance to kindergarten for out of neighborhood students occurs in August with priority is given to siblings.
