Health Requirements for 2016-17

Health Requirements for next school year have been posted. As a reminder to current and new families:

• Physical exams are required for all kindergarten, 6th and 9th grade and any child entering CPS for the first time. 
• Vision exams are required for all kinder children and any child entering CPS for the first time.  
• Dental exams are required for all kinder, 2nd and 6th grade students.

Physicals and immunization records may be submitted prior to the start of the school year, but no later than October 15, 2016. Dental exams must be completed by May 15, 2017. Please do not wait until the last minute: Students face expulsion from school until these requirements are met.
