Parent Resource Guide
This guide, assembled by parents for parents, contains important back-to-school information as well as other resources that you may reference throughout the school year.

Forms and Student Fees
Check backpacks for required forms, along with a checklist, which will be sent home by the end of the second week of school. Fees and forms are due by Monday, September 18. They may include:
1. Student Code of Conduct Book
Please read, sign and tear out the last page, and return it to the office.
2. Student Fee Form ($100 per student, $250 max per family)
This fee helps pay for school-wide curriculum materials, such as textbooks, consumable workbooks, and technology supplies—materials that are not covered by our limited school budget. Online payment is preferred, but cash or money order are also acceptable. The Student Fee Form is to be submitted with your eldest child, including name(s) and room number(s) for every student in your family, method and amount of payment. This student fee is separate from the cost of classroom supplies that students need to purchase. Payment plans are available, and a fee waiver form is available in the office if you cannot afford the fee.
3. CPS Family Income Form
This information determines annual state and federal school funding. All information provided is confidential. We need everyone to complete the form in order to help maximize funding for our school.
4. Student Health Forms Packet
Please review the minimum health requirements and due dates for immunizations, physical health, vision and dental examinations. Parents will be notified by the school nurse for any students who have not met the requirements. Visit CPS Student Health Services for more information on screening services provided, or the Office of Health and Wellness for details on advocating for students with chronic medical conditions.
5. Emergency and Health Form
If your child is a returning student, you must complete this form to ensure we have current information. New students to Waters have already completed this form during registration.
6. Media Consent Form
Pictures of our students may be used in our marketing materials and websites. Please sign and return whether or not you allow the use your child’s picture (no names are associated with pictures).

The school’s award-winning ecology program and full-time music program have been funded through the generosity of our school community for over a decade. WatersToday – our parent-run non-profit 501(c)(3) fundraising and volunteer organization—will launch their Annual Pledge Drive in October.
Field Trip Fees
Costs vary depending on events. Room parents will forward information when available.
Approximate student fees for major trips are listed below. Successful fundraising efforts can lower costs.
7th grade: approx $250 – Springfield, IL
8th grade: approx $900 – Washington D.C.
