Forms and Student Fees
The forms have arrived! Your child should be bringing them home by the end of this second week of school. Use this checklist to make sure you received everything correctly - you may also print the forms from this page by clicking on each form title below.
Please sign and return all applicable forms to your child’s teacher by Monday, September 19. Please also pay your student fees online by September 19.
1. Student Code of Conduct Book
Please read, sign and tear out the last page, and return it to the office.
2. Emergency and Health Form
If your child is a returning student, you must complete this form to ensure we have current information.New students to Waters have already completed this form during registration.
3. CPS Family Income Form
Helps determine annual state and federal school funding annually. Information provided is confidential. We need everyone to complete the form in order to help maximize funding for our school.
4. Student Medical Information
Helps track students with known medical conditions such as allergies or asthma.
5. Vision Services Form
Only fill out if you would like your child to receive a free vision exam at some point during the school year.
6.Oral Health Program Form
Only fill out if you would like your child to receive a free dental exam at some point during the school year.

6. Media Consent Form
Pictures of our students may be used in our marketing materials and websites. Please sign and return whether or not you allow the use your child’s picture (no names are associated with pictures).
7. Student Fee Form ($100 per student, $250 max per family)
This fee helps pay for school-wide curriculum materials, such as textbooks, consumable workbooks, and technology supplies—materials that are not covered by our limited school budget. If paying online is not an option you may pay by cash or money order. The Student Fee Form is to be submitted with your eldest child, including name(s) and room number(s) for every student in your family, method and amount of payment. This student fee is separate from the cost of classroom supplies that students need to purchase. Payment plans are available, and a fee waiver form is available in the office if you cannot afford the fee.
Annual Give to Waters
The school’s award-winning ecology program and full-time music program have been funded through the generosity of our school community for over a decade. WatersToday – our parent-run non-profit 501(c)(3) fundraising and volunteer organization—asks that all families make a one-time, up-front donation or a monthly pledge. The suggested pledge is $750 but any pledge amount is appreciated.
Field Trip Fees
Costs vary depending on events. Room parents will forward information when available.
Middle School student fees for major trips are listed below. Successful fundraising efforts can lower costs.
7th grade: approx $250 – Springfield, IL
8th grade: approx $900 – Washington D.C.
